Rebound Child & Youth Services is a not-for-profit, incorporated, charitable organization, providing high-quality, community-based programs and services to children, youth, and families in Northumberland County since 1997.
Rebound believes that the most effective services are those that are provided in a pro active and responsive manner; prioritizing community needs identified by families themselves. As part of that commitment, Rebound has taken the lead in the opening of a Youth Wellness Hub in Northumberland County as part of the expansion of Youth Wellness Hubs Ontario’s (YWHO) provincial initiative alongside our Collaborative Partnership Team.
About Youth Wellness Hubs Ontario (YWHO)
YWHO supports local service providers to work together to provide young people in Ontario aged 12 to 25 with low barrier access to a full range of services including but not limited to primary care, mental health and substance use counselling, service navigation, peer support and community and social supports all through youth-friendly access points.
With an anticipated opening in the summer of 2025, the Youth Wellness Hub Northumberland will offer integrated and holistic services that focus on early intervention and collaboration among various service providers in order to address gaps in the healthcare system for young people. The main goal is to improve experiences and outcomes for youth by increasing access to rapid, low-barrier services, reduce wait times for vital primary care/mental health services and reduce transportation barriers. Until the physical space is opened, staff will begin offering virtual services to community members in March of 2025.
The Hub would be the first and only Youth Wellness Hub available between Scarborough, Haliburton & Kingston ON, significantly increasing accessibility of services across the region.
About the Role
The Youth Wellness Facilitator is part of a diverse interdisciplinary Youth Wellness Team and is generally the initial point of contact for youth, families, service providers and community members reaching out to access services and gather more information about the Youth Wellness Hub Northumberland. The Youth Wellness Facilitator will work to support youth and their families accessing YWHO services. This role will support a consistent and centralized intake process to all services available through the Youth Wellness Hub Northumberland by championing the use of YWHO’s electronic client information system and screening tools. The Youth Wellness Facilitator will provide services reflective of the diverse needs of the youth and their families. The Youth Wellness Facilitator supports the coordination of the day-to-day operations of the Youth Wellness Hub Northumberland which can include liaising with partners for scheduling and programming purposes.
Main Responsibilities
Oversee the intake process for youth and families, including the use of YWHO’s electronic client information system and appropriate screening tools (My Wellness Passport)
Create and maintain a developmentally appropriate, non-judgmental, non-stigmatizing, safe, and welcoming hub atmosphere.
Represent YWHO to youth, families, and service providers, and support an orientation to the hub and an introduction to YWHO’s electronic client information system and measurement based care.
Support the transition of youth/families between the various Youth Wellness Hubs service pathways (Skills and Well-being, Community & Social Support and Clinical Services)
Collaborate with the Youth Wellness Team and other hub service providers to provide seamless access to support for youth.
Support engagement with youth and their families to support ongoing access to services and gather feedback and input onto all hub programs and services.
Provide services and support to youth and their families in a way that is consistent with the values of the YWHO Model.
Participate in YWHO Provincial Community of Practice for Youth Wellness Facilitator role.
Provide access to services both in person, remotely (off site) or virtually (online, phone) as needed.
Qualifications/Experience Required
Diploma or degree in social services or a combination of education and experience that will meet the needs of this position
Minimum 1 year experience working with children and youth in a community based mental health/substance use setting.
Experience with health information systems is an asset.
Knowledge of community resources available to support youth and families across a continuum of care and holistic understanding of health and mental health.
Knowledge of approaches that promote health equity, including anti-oppressive and anti-racist practice, 2SLGBTQ+ inclusion, community engagement, cultural safety, trauma-informed care, harm reduction, and human rights and accessibility frameworks.
Excellent organizational, and written communication skills and the ability to work effectively in a fast-paced environment are essential.
Excellent interpersonal skills in connecting/building relationships with youth, family/caregivers and service partners.
The ability to work effectively as a team member in a dynamic and fast-paced multidisciplinary environment is essential.
Experience in supporting safe and meaningful participation and being open to diverse ways of thinking about, understanding and articulating challenges related to mental health and substance use is required.
Bilingualism and/or proficiency in a second language is an asset
Valid Ontario G class Driver’s License and a reliable vehicle with minimum $2,000,000 insurance liability, as travel throughout Northumberland County is required
Must obtain a complete criminal reference check from the local Police Force, a Vulnerable Sector Search and Local Police Record Search.
Location & Commitment
Hours of Work: Flexibility in work hours will be required to best serve youth populations including ability to work weekends/evenings for purposes of program delivery and attendance at activities and events where necessary.
Location of work: Our main Hub will be located at 15 Cavan St, Port Hope, ON L1A 3B5 with future satellite spokes planned to open across Northumberland County. This main location has a planned opening for the summer of 2025. Staff will begin working out of the office spaces at Rebound Child & Youth Services (700 D’Arcy St. Cobourg, ON) and transition to the main hub once open. Staff will begin offering virtual services and transition to both in person and virtual services once the hub opens.
Travel: Candidates should be prepared to travel across satellite locations in the future and attend outreach events/initiatives where necessary.
Office Space: Candidates should expect to share their office space with our Youth Wellness Team/Community Partners through a desk hoteling model.
